$450 per person/year

Plan Exclusions & Limitations

For complete details, see Plan Terms and Conditions on our website. Youth plan is intended for ages 12 and younger. Monthly payment option will incur a 20% surcharge and a $99 enrollment fee. If a patient cancels, they will be charged a $50 re-enrollment fee if they want to sign up for the plan in the future.
